I agree to pay for consultations with Sagi Kalev (Sagi, Inc.) in advance by credit card. The fee for consultations is $100 for the initial consultation, which can be up to one hour in length and will be conducted over the telephone. Following payment, a consultation date and time will be agreed upon by Sagi and the consultee. Preferred times are Sundays from 11 a.m. to 4 p.m., but other times may be scheduled if agreeable to both parties. Consultee agrees to pay for any and all long distance or toll charges.

Discussion topics during the telephone consultations will be limited to nutrition, workout and fitness-related topics. Discussions of illegal or controlled substances and conversations of a sexual nature are prohibited. Any attempt by the consultee to discuss prohibited subjects will result in immediate termination of the consultation session and a forfeiture of the prepaid consultation fees.

Because resistance training, aerobic training and special diets can potentially have an adverse impact on some individuals with certain medical conditions, I agree to consult with a physician before implementing any recommendations made during the consultation sessions.

Consultee agrees to hold harmless and release from all liability, Sagi Kalev and Sagi, Inc., for any and all circumstances resulting from implementation of the consultation recommendations.

Consultee agrees to email Sagi Kalev, in advance of the initial telephone consultation, his or her height, weight, age, approximate bodyfat percentage, physical limitations (if any), significant medical conditions (if any), weight training and competition experience (if any), overall fitness goals, and what is expected from the consultations.

Consultee agrees not to distribute or publish by any means the information and recommendations made by Sagi Kalev during the telephone consultations. Sagi likewise agrees to keep personal information regarding the consultee confidential.
